Update 10/30/09..... Some Insurance policies have changed. You can go to any wig store of your choice, purchase the wig and submit the receipt yourself

What you need to know to be reimbursed

1. Call your insurance company to send you a form

2. Get a prescription from your doctor.....make copies

3. Diagnosis code - obtain from your doctor, type of cancer you have. 

example - breast cancer is 174.9

4. Make sure wig store/salon has their name and address on receipt

5. Submit all info - insurance companies will only pay for wigs
